There are technically six different types of items/gear which can be identified by the color of the text descriptions when you highlight them. When you salvage an item you received crafting materials that allow you to build your own items that may be more suited to your class. It's recommended that you salvage as much as you can. Items in Diablo 3 that are not usable to your class or have become useless for you based on your level or build should be salvaged or sold. If you don't pick a skill for that tier, the follower will not use either. If there are only one or two skills you want your Follower to reliably use, retrain them and pick only the skills you want them to use for the tier of skills you don't want them using, just leave the two options as they are. and is the first value from which your attack speed is determined.
The off-hand weapon seems to lend its damage and passive attributes only. That means place fast weapons on their off-hand so they do more damage.
